Okay, I think I have it. I've tested it on a static, local copy so the live behavior may have a glitch or two.

Since you're still making changes to the index page, I'm not attaching a diff file for that. Other than including the javascript 
and additional styles, it involves only adding "<div id="indexbutton" title="Hide index"><span>«</span></div>" at the 
bottom of, and within, the index-frame DIV.

I am attaching style.css.diff, indexbar.js, and indexbar.css. I hope this mail doesn't get bounced.
