How to upload image file to Wiki

I am trying to update the image file for the Inline Programming Notes page.

When I edit the page to insert a link to the non-existent file and click Preview, I am presented with a "Please upload file" 
link. Clicking on that gives me a "Please upload file" page without the Preview notification watermark. Hmm...

So, how do I upload a file? Click "Create"?
