I think it happens if debugging a project causes a crash or something.

Happens here very frequently. Of course when it happens, the project state is not saved so when re-opened it was whatever it was before.
I also note that using [File/Save Project] menu doesn't seem to avoid this. The only way to set up for debugging a touchy project is to open what i needed, close the IDE completely, open again and go.
