for index to work you must have lines with the text encased in [square brackets]

Ie.

## 2. this will not be indexed


## [3. this line will be indexed]


As for the items not showing in the right hand list,
they are showing okay here.  perhaps the page is cached on your machine. maybe a clear cache might update it.
